The Continuum of Care After Residential Treatment

Addiction treatment is most effective when it follows a structured step-down process. Rather than moving abruptly from 24/7 residential care back to independent life, the clinical continuum provides progressively less intensive support as recovery strengthens.

Step 1
Partial Hospitalization (PHP)
5–6 days per week, 5–8 hours per day. Clinical intensity of inpatient care while sleeping at a sober living home or family residence.
Step 2
Intensive Outpatient (IOP)
3–5 days per week, 3–6 hours per day. Allows return to work or school while maintaining structured therapeutic support.
Step 3
Outpatient (OP)
Weekly therapy sessions and medication management. Long-term maintenance phase with ongoing clinical oversight.

Sober Living Homes

A sober living home (SLH) is a structured, substance-free residence where people in early recovery live together. Residents pay rent, follow house rules (including regular drug testing), and support each other's sobriety. Research shows that transitioning directly from residential treatment to a sober living home significantly improves 12-month outcomes.

What to look for:

  • β€’ Regular drug testing with consistent enforcement
  • β€’ Proximity to work, IOP, and peer support meetings
  • β€’ Positive community culture

Ongoing Therapy & Medication Management

Individual therapy with a licensed counselor specializing in addiction is a cornerstone of sustained recovery. Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Motivational Interviewing all have strong evidence for relapse prevention.

If you're on Medication-Assisted Treatment (MAT) β€” buprenorphine, methadone, or naltrexone β€” continuing your medication long-term is strongly recommended. Studies show people who discontinue MAT prematurely have significantly higher relapse and overdose death rates.
